Insurance and Payment

Insurance Claim Filing Policy

Clear Brook Counseling Professionals will file your claim with the insurance company you provide to the front desk.  Should the initial claim be rejected, Clear Brook Counseling Professionals will then make any necessary adjustments to the claim and re-submit the claim for a second time.  If the second claim is also rejected, the service will then be changed to cash-pay services. At that time, the amount owed will then become the client's full responsibility. Once your services are paid for, a receipt can be generated and you can submit a claim to your insurance company for reimbursement.  

Questions to ask your insurance company BEFORE making your first appointment:

  1. Does my insurance have mental health benefits and cover therapy?
  2. What is my current deductible?
  3. Will I have a co-payment or need to pay towards co-insurance?
  4. Is there a limit on the number of sessions I can attend in a year?
  5. Do I need a referral from my primary care physician before I can see a therapist?

Payments 

Clear Brook Counseling Professionals will accept cash, check or credit/debit cards for payments. Payment or co-payment is due at the time of service. Clear Brook Counseling Professionals asks that if you have a copayment or co-insurance due at your appointment you put a credit or debit card on file to ensure timely payment. If you would like someone else to pay on your account (i.e. a parent) and you are over the age of 18, we will need a Release of Information filled out before we can talk to that person about your account. 

Cancellation Policy

We understand that things come up unexpectedly, so each client is given one free miss that will be used for their first missed appointment. A charge can be added to your account for any appointment that is missed without prior notification of at least 24 business hours. Insurance does not cover a no-show fee and $75.00 will be charged to you for the missed appointment.
